Each patient is assigned a Case Manager who will guide the patient from admission through discharge. The Case Manager is available to answer questions, assist with insurance and to plan for a safe, timely and appropriate discharge. Interdisciplinary Team Meetings are held regularly to review the patient’s progress and to set new goals. Discharge is arranged when the acute medical needs of the patient have been met and he or she is ready for the next level of care or return home.
